The Notice of Change form is intended for permanent vehicle changes only.

Once your form has been approved by The Port, you must return your current decal to the Parking Office to receive a replacement.

Decals not adherring fully may be exchanged with the permit office.

Any discrepancies or misuse of decals will result in a parking citation and possible revocation of parking privileges.
