The Notice of Change form is intended for permanent vehicle changes only.

Once your form has been approved by The Port, if the decal does not adhere properly in the new vehicle or is damaged, please return your current decal to the Parking Office for an exchange.

Any discrepancies or misuse of decals will result in a parking citation and possible revocation of parking privileges.
